Select this appointment type if you are a 1/11 Soldier who has been referred to H2F by their provider to establish care OR a 1/11 Soldier who needs a musculoskeletal condition/injury evaluated by an H2F provider

Use this appointment type if you have been instructed to by your PCM or if you have a new injury that has not been evaluated yet. If you have an injury to your elbow, wrist, or hand, make an appointment with Occupational Therapy.
Only select this appointment type if you have already been evaluated by an H2F Physical Therapist. For evaluation of a new injury, schedule a “Physical Therapy Initial Evaluation" appointment.
For all elbow, wrist, hand injury concerns, acute and chronic. Please select this appointment type if it is your first time being seen by OT. Appointments take place in Hangar 1.

1. Facility & Scheduling:
Use Lanes 1-5 ONLY. Lanes 6-16 are for H2F staff.
Max 30 participants per 2-hr block. Book multiple slots for larger groups.
Cancel 48+ hrs in advance. 2 missed reservations = 1-year ban.
2. Cleanliness & Equipment:
Leave training area & latrines clean. Remove personal items, dispose of trash & wipe down equipment.
The unit NCOIC/OIC must inspect latrines post-training. Failure results in a same-day cleaning detail.
Use AFT equipment per FM/ATP 7-22 & return to its original layout (see signage).
3. Conduct & Rules:
Do not open/close hangar doors without H2F Cadre approval. No outside music.
Be on time for your reservation and vacate promptly when finished.
May-Sep Rule: The 2-mile run must be conducted OUTSIDE unless special permission is granted by the H2F Program Director.
